Qualifications

• Master's degree in behavioral health (Counseling, Psychology, or Social Work)
• Licensed Mental Health Counselor (LMHC) or Clinical Social Worker (LCSW) in Washington
• Washington State Driver’s License
• Experience with Electronic Health Record (EHR) systems
• Strong computer skills (email, word processing, presentations, spreadsheets)

Responsabilités

• Perform brief, evidence-based, solution-focused assessments and interventions to improve functional outcomes.
• Identify, triage, and co-manage patients with medical and behavioral health needs within primary care.
• Provide psychoeducation and patient education to support behavioral change plans.
• Develop and implement behavioral change plans and protocols for target populations.
• Support primary care providers and patients with quality mental health consultations as part of the Integrated Clinical Care team.
