Job summary

The Neighborhood Outreach Access to Health (NOAH) at Cholla Health Center in Scottsdale, AZ seeks a Behavioral Health Consultant to join a multi‑disciplinary team delivering brief, consultative services to primary care patients.

Responsibilities include client screening and assessment, developing time‑limited treatment plans, coordinating with families and healthcare staff, and leveraging community resources to achieve integrated care goals.

Qualifications

• Master’s degree in Social Work, Counseling, or other behavioral health related field.
• Active licensure (LCSW/LPC/LMFT or Psychologist) in good standing in AZ.
• 1+ year of behavioral health consultative services experience.
• 3+ years of healthcare experience.

Responsibilities

• Provides full scope of consultative services including client screening and assessment and facilitating groups.
• Develops a measurable, time-limited treatment plan.
• Serves as a resource for patients, families and healthcare staff; coordinates with team members.
• Facilitates communication between family and other healthcare team members.
• Collaborates with community resources to deliver a comprehensive treatment plan.
• Maintains clinical documentation in a timely manner; participates in QA and policy activities.
• Attends meetings, trainings, and community outreach; supports continuous education.
• Performs other related duties as assigned.
