Job summary

Qualifications

• High school diploma or equivalent plus related behavioral health education/experience.
• Associate's degree and at least two years of full-time behavioral health work experience.
• Bachelor's or Master's with behavioral health focus and relevant experience.

Responsibilities

• Identify, implement, and monitor behavioral health services to meet treatment goals.
• Coordinate care with patients, families, providers, and community resources.
• Assist in applying for social security benefits and navigate Social Determinants of Health (SDOH).
• Coordinate continuity of care across levels and multiple services.
• Support outreach, reengagement after crises, and documentation in the patient’s plan.
