Job summary

Neighborhood Outreach Access to Health (NOAH) is seeking a Behavioral Health Consultant to join a multidisciplinary team delivering consultative services for primary care patients. You will screen, assess, and intervene, develop measurable treatment plans, and coordinate with families and care teams.

Ideal candidates hold a Master’s in social work or related field and a state license (LCSW/LPC/LMFT/ Psychologist). Bilingual English/Spanish is preferred.

Qualifications

• Knowledge of behavioral health services and standards.
• Excellent verbal and written communication skills and ability to interact with individuals from diverse backgrounds.
• Ability to document in electronic health records and maintain confidentiality.

Responsibilities

• Provides full scope of consultative services including client screening and assessment.
• Facilitates classes and groups and social service referrals.
• Develops a measurable, time-limited treatment plan.
• Coordinates with healthcare team to implement integrated care plans.
• Participates in audits and quality improvement activities.
