Qualifications

• Master's or Doctorate in social work from an accredited program.
• Experience or internship in a medical setting supporting primary care integration.
• Commitment to working with diverse patient populations.
• CPR certification is required.
• Knowledge of HIPAA and patient privacy regulations.
• Ability to pass a background check and have reliable transportation.

Responsibilities

• Assess behavioral health and/or substance use issues within the clinic.
• Provide brief, evidence-based treatment in the primary care setting.
• Collaborate with the care team to support patients with chronic conditions.
• Document care in the medical record with clear writing.
