Qualifications

• Licensed in RI as LICSW/LMHC/Clinical Psychologist/other approved licensure
• Experience in behavioral health or medical settings with child clinical training.
• Ability to provide telephonic and telemedicine services.
• Commitment to quality assurance and improvement processes.
• Strong communication and confidentiality practices.
• Collaborative work with diverse stakeholders.

Responsibilities

• Collaborates with primary care providers to recognize, treat and manage mental health and psychosocial issues.
• Triage referrals from the medical team and provide brief treatment or refer to appropriate resources.
• Provide brief treatment interventions for mild to moderate concerns and crisis intervention.
• Consult in real time to assist patients and families with psychosocial issues and care level decisions.
• Conduct patient care in person, via telemedicine, or telephone for consultations and treatment.
• Develop relapse prevention plans and help maintain stable functioning.
• Identify at-risk patients and plan to prevent escalation of conditions.
• Assist patients in navigating the healthcare system and coordinate with external services as needed.
• Participate in interdisciplinary care conferences and ensure open communication with the care team.
