Rhode Island Hospital is seeking an Integrated Behavioral Health Clinician to work closely with primary care teams, providing rapid behavioral health assessment, triage, and brief treatment. The role embeds within primary care practices to support patients with mental health or substance use concerns.

Responsibilities include collaboration with PCPs, triage of referrals, crisis intervention, brief treatment, telemedicine options, and education to staff.

Qualifikationen

• Licensed RI behavioral health professional (LICSW/LCPS/LMHC).
• Experience in behavioral health or medical settings with child-focused training.
• Strong communication and collaboration skills.

Aufgaben

• Collaborates with primary care providers to treat mental health and psychosocial issues.
• Triages referrals and provides brief treatment or escalates as needed.
• Delivers brief treatment and crisis intervention.
• Consults in real time to determine appropriate level of care.
• Conducts in-person, telephonic, or telemedicine sessions.
• Develops relapse prevention and navigation plans for patients.
