Home Health & Hospice Care in Merrimack, NH is seeking an Intake Support Specialist for a 24-hour-per-week role (Fri, Sat, Sun, 8:00am-4:30pm). The position coordinates referrals, verifies benefits, and maintains patient records to ensure a smooth intake process for home health and hospice services.

You will educate callers, collaborate with care teams, and provide excellent customer service while supporting multiple tasks in a fast-paced environment.

Qualifications

• High school diploma required.
• Meets background check requirements.
• Two years of medical/surgical nursing experience preferred.
• Experience with medical coding and third-party payer authorizations is preferred.

Responsibilities

• Manage incoming referrals and ensure accurate, timely processing.

About the Job:

The Intake Support Specialist plays a critical role in coordinating patient referrals and ensuring a smooth intake process for home health and hospice services. This position serves as a key point of contact for referral sources, patients, and internal teams, ensuring accurate information, timely processing, and excellent customer service.

What You'll Do:

• Route referrals appropriately to support timely start of care
• Deliver exceptional customer service to internal and external partners

Who You Are:

Preferred:

• Current RN with the New Hampshire Board of Nursing
• Minimum of two year experience in medical/surgical nursing

• ASN or BSN degree
• Knowledge of Epic EMR
