Job summary

Ecumen, a nonprofit senior living provider, seeks an Intake Specialist to manage intake and processing of new referrals, assemble clinical background information, assess referral quality, and support RN Case Managers for admission.

The role schedules full-time, 80 hours bi-weekly, M-F 8am-4:30pm with occasional weekend rotation; pay ranges $25.50-$29.00 per hour and eligibility for health, dental, vision, PTO, holidays, and mileage reimbursement.

Qualifications

• High School graduate or GED.
• Two years related experience in Medicare Home Care/Hospice coordination.
• Ability to communicate effectively in both verbal and written formats.
• Proficient with Microsoft Office Suite.
• Follow all safety rules, regulations, policies and procedures.

Responsibilities

• Coordinates the intake of patient information to ensure timely admissions, treatment, interventions and resource utilization in compliance with policies, procedures and regulatory requirements ensuring timely and effective patient care.
• Verifies patient payer sources and schedules all hospice and home care admissions in software system and ensures accuracy of information.
• Responsible for coordination of all patient care; coordinates care and services with other departments/providers from the time of referral to discharge.
• Assumes added responsibility for special projects, administrative functions as directed and follows through with timely completion of the project.
• Performs other duties as assigned.

Job description

The Intake Specialist position is responsible for the intake and processing of new referrals in accordance with organization standards. This position is responsible for assembling information concerning a patient's clinical background and referral needs, assesses quality of referral documents and provides appropriate clinical information to RN Case Managers for admission.

Opportunity Details:

• Schedule: Full-Time 80hrs bi-weekly - M-F 8am-4:30pm - Weekend rotation on Saturdays 8am-4:30pm for a short time
• Pay: The targeted pay range for this job is $25.50-$29.00 per hour.

Minimum Required Qualifications:

• High School graduate or GED
• Two years related experience in Medicare Home Care/Hospice coordination
• Ability to communicate effectively in both verbal and written formats
• Proficient with Microsoft Office Suite
• Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, Resident's Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and MSDS

Preferred Qualifications:

• Licensed Practice Nurse or Bachelor's Degree in Health Information Management
