Home Health & Hospice Care in Merrimack, NH, is seeking an Intake Specialist for a part-time, benefits-eligible role. The position focuses on coordinating patient referrals, validating insurance, and ensuring smooth intake processes for home health and hospice services.

You will be a primary contact for referral sources, patients, and internal teams, providing accurate information and timely processing while delivering excellent customer service and maintaining EMR records.

• Collaborate with care teams, liaisons, and departments to coordinate admissions.
• Route referrals appropriately to support timely start of care.
• Deliver exceptional customer service to internal and external partners.
• Maintain productivity standards and contribute to a team-oriented environment.

Preferred:

• Current RN with the New Hampshire Board of Nursing
• Minimum of two year experience in medical/surgical nursing
• Minimum of one year experience in medical coding and third-party payer authorizations
• ASN or BSN degree
• Knowledge of Epic EMR
