Community Living, Inc. is seeking a Program Intake and Services Coordinator who serves as the first point of contact for families, case managers, school personnel, and other stakeholders.

You will handle case coordination, individual plans, behavior management, daily documentation, and semi-annual reporting. You will train staff on individual and behavior support plans, ensure safety, and manage daily operations across the SOAR program.

Formación

• 18 years of age or older.
• Bachelor’s degree in a social services field or related field preferred.
• Two years of experience working with individuals with intellectual and/or developmental disabilities preferred.
• One year management or case management experience preferred.
• Possess sound oral and written communication skills as well as decision-making skills.
• Must be able to write and document in a clear, concise, and professional manner.
• Ability to make decisions and react to change quickly.
• Possess the ability to use Microsoft Office applications including Outlook, Word, and Excel.

Responsabilidades

• Manage the SOAR priority lists.
• Schedule intake meetings, transition visits, and family communication.
• Tour and observe facilities to ensure quality services.
• Coordinate the individual planning process.
• Review documentation monthly and update data collection methods as needed.
• Assess and determine support needs for each participant served.
• Provide feedback and coaching for staff on daily documentation expectations.
• Review all incidents and injuries for proper documentation and provide feedback to Team Leads.
• Ensure the safety and welfare of all participants.
• Pass medications without error.
• Represent Community Living and the SOAR program at resource fairs and community events as needed.
• Complete and maintain all required trainings.
• Perform other duties as required and assigned.
