Job summary

Peace River Center in Lakeland, FL is seeking a Care Coordinator Non-Targeted Case Manager to coordinate services for High Need/High Utilizer clients and ensure continuity of treatment during psychiatric care. You will assist clients in accessing post-discharge services and educate and empower them throughout the process.

Use SOAR to assist applicants for Social Security benefits, complete required documentation, and maintain monthly reports for the care coordination caseload.

Qualifications

• Bachelor's Degree in a Human Services field from an Accredited College or University.
• Case Management and SOAR Training completed with supervisor upon hiring.
• Experience preferred but not required.

Responsibilities

• Coordinate services and cross-system collaboration for High Need/High Utilizer clients.
• Provide discharge planning and linkages to HNHU clients at CSUs.
• Manage caseload of 20 HNHU clients and report monthly to Central Florida Behavioral Health Network.
• Provide transportation to participants for medical, outpatient, dental and social security appointments.
