Job summary

Community Living in Missouri is seeking a dedicated Intake and Services Coordinator for the SOAR program. You will be the first point of contact for families, case managers, and school personnel, handling inquiries, referrals, transitions, and program entry.

The role includes coordinating individual plans, behavior support, daily documentation, and semi-annual reporting while training staff on plans and ensuring participant safety. Full-time, 40 hours/week, $24.25/hr.

Qualifications

• Bachelor's degree preferred in a social services or related field.
• Two years of experience working with individuals with intellectual and/or developmental disabilities preferred.
• One year management or case management experience preferred.

Responsibilities

• Manage SOAR priority lists and scheduling of intake meetings and family communication.
• Coordinate the individual planning process and quarterly/ semi-annual reporting.
• Provide feedback and coaching for staff on daily documentation and incident reporting.
• Ensure safety and welfare of all participants and assist with transportation as needed.
• Complete required trainings and participate in community events as needed.
