SOAR365 is hiring a Case Manager to coordinate services for participants across Day Support, Respite, Summer Camp, and Employment Services. You will review intake materials, develop individualized service plans, and monitor progress with participants, families, and support coordinators.

You will also complete annual assessments, keep thorough records, and advocate for participants in planning meetings, ensuring services align with person-centered goals and community resources.

Qualifikationen

• Knowledge of community resources and human services.
• Ability to interview, counsel, and collaborate with families and stakeholders.
• Familiarity with SOAR365 programs, policies, and regulatory requirements.

Aufgaben

• Coordinate and manage services for participants across multiple programs.
• Review intake docs and develop ISPs with individuals and representatives.
• Complete annual assessments and quarterly reports.
• Maintain accurate documentation and monitor ISP progress.
• Advocate as primary contact for participants and their networks.
