Job summary

ProHealth Group seeks an Intake Coordinator for ProHealth Home Health & Hospice in Centre, AL. This role supports business development and operations by entering patient referrals and coordinating with care teams to initiate care promptly.

The coordinator will handle multiple referral platforms, maintain accurate documentation, and communicate with hospitals, physicians, and patients to ensure seamless care transitions.

Qualifications

• High School Diploma or GED is required.
• One year of data entry experience.
• One or more years of intake experience preferred.

Responsibilities

• Enter patient referrals promptly with required details (name, DOB, address, phone, SSN, insurance, referral source).
• Review clinical information and notify the appropriate coordinator for additional data.
• Notify staff of receipt and status of referrals in the referral system.
• Enter a communication note on every referral for traceability.
• Handle fax, email and multiple electronic referral platforms (Navi Health, EnsoCare, Watershed, AllScripts).
• Maintain timely acceptance/decline of referrals and share updates with CTCs and managers.
• Prepare daily intake reports and distribute to relevant staff.
• Complete timesheets and submit by Monday 9am to the Director of Business Development.
