GEO Reentry Services LLC in Campbell, CA is seeking a dedicated professional to coordinate placements for program participants transitioning from CDCR facilities to CBP-supported programs.

You will collaborate with Parole Officers, Transportation Coordinators, and CBP providers to assess needs, track referrals, arrange transportation, and ensure participants have medications and supplies before entry, while completing required reports for CDCR and the Program Manager.

Qualifications

• High school diploma or equivalent.
• Experience with case management and criminal justice populations preferred.
• Familiarity with cognitive behavioral therapy and addiction theory.
• Understanding of California treatment initiatives and reentry care.
• Strong written and verbal English communication skills.

Responsibilities

• Coordinate participant placements from CDCR locations to CBP programs.
• Collaborate with Parole Officers to determine placements.
• Coordinate transportation to program placements.
• Track referrals and conduct assessments to match needs.
• Assist with reception into programs with CBP providers.
• Prepare required reports for CDCR and Program Manager.
• Respond to concerns and communicate with CDCR when non-placements occur.
• Ensure participants have needed medication or supplies before entering facilities.
