Qualifications

• Licensed pharmacist in the employment state.
• Minimum 2 years’ experience in home infusion pharmacy preferred.
• Experience with intravenous admixture and IV therapy.
• Pharmacist education credentials as listed (PharmD preferred).
• Desirable membership in professional societies.

Responsibilities

• Oversee pharmacy staff including pharmacists and technicians.
• Perform intake pharmacy activities: chart reviews, dosing, and order creation.
• Coordinate operations with other departments to ensure efficient workflows.
• Ensure documentation and compliance with URAC/ACHC accreditation requirements.
• Provide clinical consultations to physicians and other healthcare professionals.
