Job summary
Option Care Health in Irving, Texas is seeking a licensed Clinical Pharmacist to provide safe and effective pharmacy services for patients in a home infusion setting. This entry-level role coordinates with the Nursing Department, educates patients and caregivers, and ensures adherence to professional standards and regulations.
The position involves designing pharmacy care plans, monitoring outcomes, and supporting medication safety in the home care continuum.
Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- Experience or training in sterile IV admixture, MTM, or residency (see preferred qualifications).
Responsibilities
- Assess patients for home care suitability and collect patient information.
- Design and monitor patient-specific pharmacotherapy plans for home care.
- Modify care plans based on monitoring data and lab values.
- Educate patients, caregivers and staff on medication use.
- Ensure safe dispensing, labeling and storage of medications.
- Maintain equipment and inventory accuracy for home infusion.
- Respond to drug information requests and participate in CEU activities.
- Adhere to all regulatory, safety and quality standards.
