Option Care Health in the United States seeks an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services in the home infusion setting. You will coordinate patient services with Nursing, monitor progress toward goals and educate patients, caregivers, staff and professionals on medication use.

The role emphasizes drug information, IV admixture, and ongoing CEU participation; strong attention to safety, legal compliance and quality assurance are expected.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of: at least 1 year sterile IV admixture experience or MTM knowledge or ASHP residency or APPE or equivalent experience.

Responsibilities

• Assess home care suitability and collect patient information.
• Design and monitor patient-specific pharmacy care plans for home care patients.
• Modify care plans based on monitoring data and lab values.
• Provide medication-use education to staff, patients and caregivers.
• Ensure continuity of pharmaceutical care across home and other settings.
• Operate and maintain medication equipment and inventory with safety and accuracy.
• Respond to drug information requests and pursue CEU opportunities.
