Job summary

Option Care Health is a leading home and alternate-site infusion services provider delivering safe, high-quality pharmacy care to patients across the United States. This entry-level Clinical Pharmacist role coordinates patient services with nursing, educates patients and caregivers, and ensures appropriate medication use in the home infusion setting.

The successful candidate will be a licensed PharmD with a strong foundation in IV admixture, MTM, and compliance with DEA regulations, supported by

Qualifications

Graduate of an accredited School of Pharmacy.
Current, active Pharmacy license as granted by the State Board of Pharmacy.
One of the following: (a) sterile IV admixture experience, (b) non-sterile compounding/immunizations/MTM, (c) ASHP residency, (d) APEX program or equivalent experience.

Responsibilities

Determines the suitability of individual patients for home care and collects necessary information.
Designs and monitors patient-specific pharmacy care plans for home care therapy.
Modifies care plans based on monitoring data and evaluates laboratory values.
Provides medication-use education to staff, patients, and caregivers.
Ensures continuity of pharmaceutical care across home and other settings.
Operates and maintains medication administration equipment and completes QA documents.
