Enhabit Home Health & Hospice is seeking a referrals coordinator to streamline patient referrals and serve as a key liaison with referral sources. You will work with branch and sales teams to ensure smooth transitions to services and uphold Medicare guidelines.

The role requires a HS diploma, 2+ years in healthcare (1+ year in home health or pediatrics), strong customer service, and tech savvy with mobile devices. Valid driver license and reliable transportation are included in the prerequisites.

Qualifications

• Must have a high school diploma or equivalent.
• Minimum two years of experience in health care and one year in home health or pediatrics.
• Experience in customer service is required.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills including operation of a mobile device.

Responsibilities
