Job summary

Enhabit Home Health & Hospice is seeking a Referral Coordinator to support patient referrals and act as a resource for referral sources. You will collaborate with branch and sales teams to ensure smooth transitions to services and apply knowledge of regulatory guidelines.

The ideal candidate has a high school diploma, 2+ years in healthcare, 1 year in home health or pediatrics, strong customer service skills, and basic technology proficiency with mobile devices.

Qualifications

• Must possess a high school diploma or equivalent.
• Two+ years of experience in health care and one year in home health or pediatrics.
• Demonstrated experience in customer service.
• Knowledge of Medicare guidelines that govern home health or hospice.
• Basic technology skills, including operation of a mobile device.
• Education and experience preferred: RN or LPN licensed in the state.
• Experience with intake responsibilities and multi-location company preferred.
• Requirements: valid state driver's license, automobile liability insurance, dependable transportation; driving requirements may vary by location.

Responsibilities
