Job summary

Enhabit Home Health & Hospice is seeking a coordinator to manage patient referrals and act as a liaison for referral sources. You will collaborate with branch and sales teams to coordinate care and ensure smooth transitions to services.

The role requires experience in healthcare and knowledge of Medicare guidelines, with strong customer service and basic tech skills. A valid driving license and reliable transportation are expected where applicable.

Qualifications

• Must have high school diploma or equivalent.
• At least two years of healthcare experience and one year in home health or pediatrics.
• Experience in customer service.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
