Job summary

Enhabit Home Health & Hospice seeks a dedicated healthcare professional to join our team as a field-based coordinator. You will coordinate patient referrals, collaborate with branch and sales teams, and ensure smooth transitions to services while adhering to Medicare guidelines.

The ideal candidate will have at least two years in health care, strong customer service skills, a valid driver's license, and the ability to operate mobile devices.

Qualifications

• High school diploma or equivalent required.
• Experience in health care industry with home health or pediatrics preferred.
• Demonstrated knowledge of Medicare guidelines that govern home health or hospice.
• Good customer service and communication skills.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
