Job summary

Enhabit Home Health & Hospice is seeking a coordinated, customer-focused professional to manage patient referrals and act as a knowledgeable resource for referral sources. You will work with branch and sales teams to ensure smooth transition to services while applying Medicare guidelines and regulatory standards.

Applicant should have a high school diploma, 2+ years in health care (1+ year in home health or pediatrics), strong customer service skills, and comfort with mobile technology.

Qualifications

• Must have a high school diploma or equivalent.
• Two years of experience in the health care industry and one year in home health or pediatrics are required.
• Demonstrated experience in customer service.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic demonstrated technology skills, including operation of a mobile device.
