Job summary

Cornerstone Caregiving is expanding its team in Billings, MT, seeking an Intake Coordinator to guide prospective clients from inquiry through start of care. You will manage intake follow-up, initial assessments, and ongoing reassessments, coordinating with scheduling, recruiting, and leadership to ensure a smooth handoff for new clients.

The role includes coordinating long-term care insurance processes, documenting activities in AxisCare, and maintaining accurate records while building trusted

Qualifications

• Strong communication with older adults, families, and professionals.
• Excellent organization, follow-through, documentation and time-management.
• Ability to discuss sensitive care needs with discretion and empathy.
• Familiarity with HIPAA, policy requirements, and intake documentation standards.

Responsibilities

• Lead new client intake and follow-up with prompt, professional communication.
• Perform in-home, hospital, or facility assessments and collect required documentation.
• Create initial service plans and coordinate start-of-care handoffs with scheduling leadership.
• Assist clients with long-term care insurance coordination and documentation.
• Maintain relationships with referral partners and provide timely status updates.
• Coordinate with Scheduling and Recruiting to ensure feasible care delivery and appropriate caregiver assignments.
