Graham Healthcare Group in Troy, MI is hiring an Intake Coordinator - Weekends Coverage to join our dynamic team. The role interfaces with referral sources and is responsible for receiving referrals and understanding the clinical and service needs of patients.

The position requires Homecare/Home Health intake experience, Saturday/Sunday coverage, and excellent customer service skills. Associate degree preferred with at least one year in home health or similar setting.

Qualifications

• Experience in home health or hospice intake is preferred.
• Customer service experience in a healthcare setting is required.
• Strong data entry and referral processing skills.

Responsibilities

• Process home health referrals accurately and timely.
• Complete basic diagnosis coding per referral information.
• Coordinate care for patients with clinical staff.
• Determine referrals with clinical intake team.

This position requires coverage every Saturday and Sunday

Compensation: $20.00 - $24.00

The base compensation range for this role is fixed, with a maximum cap of $24.00. We want to be transparent about this as we continue discussions.

Hours: 6:30AM - 5PM EST, Thursday - Sunday, 4X10's

Graham Healthcare Group is hiring an Intake Coordinator - Weekends Coverage - to join our dynamic team! The Intake Coordinator - Weekends Coverage will interface with the Agency’s referral sources and is responsible for receiving the referrals and understanding of all the clinical and service needs of the Agency’s patients. The Intake Coordinator position works with other members of the Intake team to ensure optimal patient care and timely service by our clinical staff.

Home Health Intake Coordinator Responsibilities:

• Responsible for processing agency home health referrals in an accurate, timely manner including accurately completing data input and client information as listed on the referral.
• Complete basic diagnosis coding per referral information
• Coordinate care for homecare patients while using the Welcome Call Script
• Ability to determine referrals from the Clinical Intake team.

Home Health Intake Coordinator Qualifications:

• Associate’s Degree Preferred
• At least one year experience in Home Health preferred
• At least one year of experience in a customer service capacity
