Job summary

Choice Health At Home in Riverview, FL is seeking an Intake Coordinator for the Home Health Branch. This full-time role manages initial patient referrals and acts as the primary liaison between hospitals, physicians, families and clinical staff to ensure a seamless start of care.

You will document referrals, verify payor sources, obtain authorizations, and accurately enter patient data into the EMR. Strong computer and customer service skills are essential; LVN/LPN is preferred.

Qualifications

• Minimum one year of experience in referral intake is preferred.
• Experience with third party payers, Medicare and Medicaid highly desirable.
• Medical field background preferred, including familiarity with medical terminology.
• Excellent computer and customer service skills.
• LVN/LPN preferred.
• Valid Driver’s License and valid Auto Insurance.

Responsibilities

• Receives and appropriately documents new referrals in a timely manner.
• Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications.
• Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.
• Provides effective communication to patients, staff members, other health care professionals and referral sources.
• Responds to Agency and patient needs in a professional and creative manner.
• Demonstrates commitment, professional growth and competency.
• Participates in Agency-sponsored in-service training.
• Participates in the Agency’s QAPI program.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.
