Choice Health at Home is seeking a Home Health Intake Coordinator to manage initial patient referrals and serve as the primary liaison among hospitals, physicians, families, and clinical staff to start care smoothly. The role requires at least one year in referral intake and familiarity with third-party payers, Medicare/Medicaid, and medical terminology.

Proficiency with EMR systems and strong customer service skills are essential.

Qualifications

Minimum one year of experience in referral intake or similar setting.
Experience with third party payers, Medicare and Medicaid helpful.
Medical field background preferred.
Excellent computer and customer service skills.
LVN/LPN preferred.
Valid driver’s license and auto insurance.
HomeCare HomeBase experience preferred.

Responsibilities

Manages the initial patient referral process between hospitals, physicians, families, and clinical staff to ensure a seamless start of care.
Receives referrals, verifies insurance benefits, obtains authorizations, and enters patient data into EMR.
Verifies payor source and tracks insurance authorizations and initial certifications.
Coordinates care with Nursing Supervisors, patients, and the interdisciplinary team.
Communicates effectively with patients, staff, other health professionals, and referral sources.
Promotes agency policies and participates in in-service training and QAPI initiatives.
