Brighton Hospice in Minnesota is seeking an Intake Coordinator to join our compassionate team. You will answer inquiries, track referrals, and support chart reviews while maintaining high-quality patient care.

Qualifications

• Minimum of two years’ experience in hospice or health care.
• Associate degree or equivalent college credential preferred.
• Proficient with Microsoft Word and Excel.
• Strong telephone and customer service skills.

Responsibilities

• Answers telephone inquiries and channels them appropriately
• Assists with tracking and processing referrals
• Assist with audits and chart reviews as assigned
• Uploads documents to electronic medical records system
• Provides feedback on process and flow improvement
• Assists with scheduling goals of care and admission visits
• Work with sales team to receive incoming referral information
• Assists on weekends/holidays as needed
• Other duties as assigned by Director of Intake
