The Intake Coordinator is an exciting opportunity to support our established and amazing Home Health & Hospice team! The ideal candidate is professional, organized, home health and hospice savvy, compassionate, experienced working with a team, driven, proactive, quality focused, creative, fun (!), technologically adept, positive, and overall awesome. The Intake Coordinator will be responsible for:

• All aspects of the patient intake process
• Establishes and maintains positive working relationships with current and potential referral sources
• Ensures compliance with all State and Federal referral/intake regulatory requirements
• Ensures seamless transition of patients to home care
• Builds and monitors community relationships
• Potential for cross-training in medical records and scheduling
