You will serve as the first point of contact for patients and referral partners, ensuring timely admissions, verifying insurance, coordinating care services, and delivering a seamless transition into home health. You will:* Oversee all daily patient intake and referral operations* Manage the full intake process, including insurance verification and authorization* Coordinate and manage patient scheduling to ensure timely admissions, evaluations, and home health visits* Optimize clinician schedules while balancing patient needs, geographic considerations, and staffing resources* Serve as a key point of contact for patients, families, physicians, and referral sources* Receive, process, and track incoming referrals to support efficient patient onboarding* Communicate with clinical and administrative teams to facilitate smooth transitions into care* Monitor scheduling changes and proactively resolve conflicts, delays, or coverage concerns* Track and report referral data, trends, and performance metrics* Identify opportunities to improve services and patient access to care***Our Commitment to You**** Perform meaningful work focused on improving patient care and outcomes* Work with a collaborative and mission-driven team that celebrates your success* Ongoing professional development and training***Our Requirements**** High school diploma or equivalent required* Minimum of one (1) year of experience in healthcare scheduling, intake, medical office administration preferred* Home Health experience a plus* Excellent communication, customer service, and relationship-building skills* Knowledge of healthcare regulations and business operations* Current CPR certificationThe employer for this position is stated in the job posting.
