Vivie is seeking a Clinical Intake Coordinator to manage referrals, document patient data, and support admissions and care teams. The role emphasizes collaboration with Sales and care staff to ensure timely service delivery and accurate information flow.

Qualifications

• LPN licensed in Minnesota and North Dakota.
• Strong organizational and data accuracy skills.
• Excellent communication and customer service abilities.
• Ability to pass state-mandated background checks.
• English reading/writing/speaking proficiency.

Responsibilities

• Manage end-to-end referral process from physicians, hospitals and other sources; assess eligibility and ensure timely data collection.
• Nurture referrals and follow up with patients, families and sources to guide pre-admission processes.
• Ensure accurate documentation and EMR data entry in compliance with HIPAA and standards.
• Collaborate with Sales, Outreach and operations to smooth transitions and continuity of care.
• Serve as the first point of contact, triaging inquiries with empathy and providing guidance on services and eligibility.
