Job summary

Vivie is seeking a Clinical Intake Coordinator in Minnesota to ensure prompt, accurate, and compassionate service for clients. You will manage referrals, capture high-quality data, and support home and community-based services, contributing to timely care delivery and operational excellence.

You’ll handle end-to-end referrals, follow up with patients and providers, document in EMR per HIPAA, and collaborate across Sales, Outreach, and care teams.

Qualifications

• This role requires LPN licensure in Minnesota and North Dakota.
• Strong organizational skills and attention to detail are essential.
• Excellent communication and customer service abilities are needed.

Responsibilities

• Manage end-to-end referral process from physicians, hospitals, and other sources.
• Maintain regular contact with patients, families, and referral sources.
• Verify and enter patient data into EMR in line with HIPAA.
• Collaborate with Sales, Outreach, and operational teams to ensure smooth transitions.
• Serve as a trusted first point of contact for prospective clients and professionals.
