• Manage the hospice intake process from initial referral through patient admission
• Coordinate with physicians, hospitals, skilled nursing facilities, and care teams to gather clinical information
• Communicate compassionately with patients, families, and caregivers to explain hospice services and eligibilityVerify insurance benefits, including Medicare, Medicaid, and private insurance for hospice coverage
• Maintain accurate and timely documentation within the electronic medical records (EMR) system
• Collaborate with clinical and administrative staff to ensure seamless care transitions
• Track and follow up on pending referrals to maximize timely patient enrollment

Requirements:

• Previous experience in hospice, home health, or healthcare intake/admissions strongly preferred
• Familiarity with hospice regulations, Medicare/Medicaid eligibility, and insurance verification
• Excellent communication and interpersonal skills with a compassionate, patient-centered approach
• Strong organizational skills with the ability to manage multiple referrals simultaneously
