Job Description
As the first point of contact for patients, the Intake Coordinator plays a key role in creating a welcoming, organized, and efficient healthcare experience. This position combines administrative coordination with basic clinical support responsibilities, ensuring patients receive the care and resources they need while helping healthcare teams deliver outstanding service. The Intake Coordinator manages patient intake, conducts standardized screenings, maintains accurate records, and supports the overall flow of daily operations.
Key Responsibilities
- Welcome patients and visitors in person and support telehealth appointment check-ins.
- Manage patient check-in and check-out processes.
- Collect co-pays and other required payments.
- Verify and gather patient paperwork, insurance information, and medical records.
- Maintain accurate demographic, medical, and appointment information in electronic records.
- Ensure all documentation is completed correctly and filed appropriately.
- Schedule future appointments and support a seamless patient experience.
- Conduct standardized screenings and assessments in accordance with organizational protocols.
- Administer tools such as PHQ-2, PHQ-9, GAD-7, PRAPARE, GPRA, tobacco cessation screenings, and other approved assessments.
- Document screening outcomes accurately within the electronic health record.
- Communicate high-risk screening results to healthcare providers and care team members promptly.
- Provide approved educational materials to patients.
- Coordinate referrals and connect patients with resources to address identified social needs.
- Assist with provider-directed follow-up tasks.
- Answer questions related to appointments, services, and required forms.
- Partner with physicians, nurses, social workers, care coordinators, and other healthcare professionals.
- Maintain strict patient confidentiality and compliance with healthcare privacy standards.
- Participate in ongoing learning opportunities and professional development initiatives.

- Understanding of medical terminology and basic healthcare practices.
- Knowledge of HIPAA regulations and patient privacy requirements.
- Familiarity with health insurance processes and procedures.
- Associate degree in Health Administration, Medical Assisting, or a related field.
- Bilingual skills, particularly Spanish.
- Experience working in a community health setting.
- Understanding of social determinants of health and available community resources.
