What You’ll Do

• Receive referrals by phone or in person for the designated department
• Explain benefits to patients, families, and community contacts
• Assist in finding alternative services if the patient is not a hospice candidate
• Oversee scheduling of initial nursing assessments and admissions
• Maintain admission packets and ensure proper forms are available to staff
• Assist community outreach with marketing activities to promote referral sources
• Complete admission paperwork as needed
• Maintain punctuality and attendance
• Provide telephone and clerical support to staff as needed
• Maintain accurate clinical records, tracking client goals and progress

Qualifications

• Be 18 years or older
• At least one year of experience in homecare, hospice, or healthcare preferred
• Ability to complete documentation accurately and timely
• Handle stressful and delicate situations
• Strong interpersonal, verbal, and written communication skills
• Valid driver’s license and proof of insurance
