What You’ll Do
- Receive referrals by phone or in person for the designated department
- Explain benefits to patients, families, and community contacts
- Assist in finding alternative services if the patient is not a hospice candidate
- Oversee scheduling of initial nursing assessments and admissions
- Maintain admission packets and ensure proper forms are available to staff
- Assist community outreach with marketing activities to promote referral sources
- Complete admission paperwork as needed
- Maintain punctuality and attendance
- Provide telephone and clerical support to staff as needed
- Maintain accurate clinical records, tracking client goals and progress
Qualifications
- Be 18 years or older
- At least one year of experience in homecare, hospice, or healthcare preferred
- Ability to complete documentation accurately and timely
- Handle stressful and delicate situations
- Strong interpersonal, verbal, and written communication skills
- Valid driver’s license and proof of insurance
