Job summary

Northwestern Medicine in Winfield, IL seeks an Intake Evaluator Case Therapist to assess patients across outpatient behavioral health, ED, and medical units. You will conduct assessments, plan treatment, provide direct services, and coordinate care with treatment teams and families.

Responsibilities include documentation in the medical record, discharge planning, and on-call coverage as needed. Requires a master's in social work or related field and LPC/LSW licensure within 12 months.

Qualifications

• Master’s degree in social work, psychology, counselling, or related field.
• Licensed Professional Counsellor (LPC) or Licensed Social Worker (LSW) within twelve months of hire.
• Two years post graduate experience; experience with substance abuse and mental health populations.

Responsibilities

• Provides information and updates to patients and customers promptly.
• Demonstrates teamwork across departments and effective communication.
• Manages work schedule efficiently and meets deadlines.
• Documents in patient EMR via Epic or other systems accurately and timely.
• Assesses patients and develops therapeutic interventions.
• Communicates with patient and support system about admission, stay, and discharge planning.
• Maintains documentation per CDH standards and regulatory requirements.
• Completes intake evaluations and justifies level of care using placement criteria.
• Willing to be on-call and adapt to shifts as needed.
