Job summary

Northwestern Medicine Central DuPage Hospital in Winfield, IL is seeking an Intake Evaluator Case Therapist for Behavioral Health Services. Schedule: Mon-Fri 1:00 PM–9:00 PM.

Assess patients for mental health and substance use concerns in outpatient and hospital settings, with duties including treatment planning, direct patient services, and discharge planning. Requirements include a Master's degree in a related field and licensure within 12 months (LPC/LSW).

Qualifications

• Master's degree in social work, psychology, counselling or related field.
• Licensed Professional Counsellor (LPC) or Licensed Social Worker (LSW) within twelve months of hire.
• Two years post graduate experience; experience with substance abuse and mental health populations.

Responsibilities

• Provides accurate information and timely updates to patients and customers.
• Demonstrates teamwork by collaborating across departments and respecting diverse opinions.
• Manages work schedule efficiently and completes tasks on time.
• Documents in the patient electronic medical record (Epic or equivalent) accurately and timely.
• Retrieves patient information from hospital medical records to support continuity of care.
• Develops therapeutic interventions based on DSM diagnoses with input from the treatment team.
• Communicates with patient and support system about admission, stay, and discharge planning.
• Documents in medical records according to CDH standards and regulatory guidelines.
• Conducts intake evaluations/assessments and justifies level of care using placement criteria.
• Responds to shifts in patient needs and accepts on-call work as required.
