Job summary

Qualifications

• Master’s degree in Social Work, Psychology, Counselling, or related field.
• Experience working with substance abuse and mental health populations.
• Licensed Professional Counsellor (LPC), Licensed Social Worker (LSW) within twelve months of hire.

Responsibilities

• Assess patients for acute mental health crisis and substance concerns in ED and medical units.
• Develop treatment planning and provide direct patient services including counseling when needed.
• Coordinate care with treatment team, patient, and family; document in medical record.
• Discharge planning and continuum of care documentation.
• Communicate with physicians and departments; maintain patient rapport and collaboration.
