Job summary

Northwestern Medicine in DeKalb, IL is seeking a Behavioral Health Intake Assessor/Case Therapist II for a full-time role. The position requires a Master’s in behavioral health and independent licensure, with two years of supervised clinical experience in a behavioral health setting.

Qualifications

• Master’s degree in behavioral health.
• Two years supervised clinical experience in behavioral health.
• Licensed to practice independently (LCPC, LCSW, LMFT, APN, or Clinical Psychologist).

Responsibilities

• Complete Integrated Assessments and Treatment Plans in alignment with Medicaid and rule 2060 requirements.
• Make appropriate clinical recommendations for clients with mental health and addictions needs based on presenting symptoms.
• Assess and evaluate client needs, strengths, goals, and resources.
• Work with an interdisciplinary team in reviewing cases and developing individual plans.
• Consult with other center staff regarding problem assessment and treatment planning.
• Consult with other service agencies around case-specific and programmatic issues.
• Refer and link clients to other human services programs.
• Serve as a liaison to other agencies as needed.
• Create clinical documentation in support of services provided on a timely basis.
• Train other providers and non-providers in areas relating to service specialties.
• Functions cooperatively as a member of the treatment team.
