Job summary

Northwestern Medicine in DeKalb, IL seeks a Behavioral Health Intake Assessor/Case Therapist II for a full-time, 40-hour work week. Typical hours are Monday–Friday with two evening shifts until 8:30 PM to support patient needs.

Qualifications

• Master’s degree in behavioral health is required.
• Two years supervised clinical experience in behavioral health.
• Licensed to practice independently (LCPC, LCSW, LMFT, APN or Clinical Psychologist).
• All NM employees must have a valid Illinois driver’s license, auto liability insurance, and annual driving abstract.

Responsibilities

• Complete Integrated Assessments and Treatment Plans per Medicaid and Rule 2060 requirements.
• Make appropriate clinical recommendations for clients with mental health and addictions needs.
• Assess and evaluate client needs, strengths, goals, and resources.
• Work with an interdisciplinary team to develop individual plans.
• Consult with center staff on problem assessment and treatment planning.
• Refer and link clients to other human services programs.
• Serve as a liaison to other agencies as needed.
• Create clinical documentation in support of services provided timely.

Education

Master’s degree in behavioral health

Two years supervised clinical experience

Licensed to practice independently

Licensed Clinical Professional Counsellor (LCPC) or Licensed Clinical Social Worker (LCSW) or Licensed Marriage & Family Therapist (LMFT) or APN or Clinical Psychologist
