Job summary

Phoenix Rescue Mission in Arlington, TX seeks an organized intake coordinator to support the RAP Program. You will conduct screens, complete intake appointments, and arrange case management while ensuring accurate data entry and adherence to program policies.

Ideal candidates bring at least 1 year in social services, strong Microsoft Office skills, and a compassionate, professional demeanor in emergencies. A Christian approach and teamwork are valued in this role.

Qualifications

• High school diploma or GED required; bachelor's degree preferred in a related field.
• Minimum of one year of experience in social services, intake, behavioral health, customer service, or related field.
• Previous experience in intake, scheduling, and coordination of care preferred.
• Certification in Peer Support is highly desired.
• Proficient in Microsoft Office software.
• Program database experience preferred.

Responsibilities

• Conduct phone screens to determine prospective clients' eligibility for the RAP Program and gather detailed information.
• Complete intake appointments with required documentation, obtaining releases and consent forms, and data entry.
• Schedule case management appointments and follow-up services for smooth transitions into care.
• Conduct property searches and urine analyses per program policies.
• Connect clients with community resources and coordinate external referrals when different levels of care are needed.
• De-escalate crisis situations and communicate effectively with clients, families, and team members.
• Build collaborative relationships with community providers and internal teams to ensure continuity of care.
• Comply with legal, regulatory, and ethical requirements and follow the Mission's policies and Code of Ethical Conduct.
• Follow the Mission’s Statement of Faith and participate in organizational activities that reflect a Christian witness.
• Perform other duties within scope as assigned.
