Job summary

Phoenix Rescue Mission in Phoenix, AZ is seeking a compassionate intake specialist to screen clients for eligibility, gather documents, and coordinate care within the RAP Program. You will work with clients, families, and partners to support placement and access to services.

The role requires strong organization, empathy, and teamwork, with a commitment to the Mission’s Christian values. Experience in social services and data entry are preferred.

Qualifications

  • Minimum one year in social services, intake, behavioral health, or related field.
  • Bachelor’s degree in social work, sociology, psychology or related field preferred.
  • Proficiency with Microsoft Office and client data entry.

Responsibilities

  • Conduct intake and screening to determine eligibility for RAP program.
  • Schedule and coordinate case management appointments and follow-ups.
  • Collect required documentation and ensure timely data entry into the database.
  • Coordinate referrals and connect clients with community resources.
  • De-escalate crises and communicate effectively with clients and teams.
  • Maintain compliance with policies and code of ethical conduct.
  • Support devotions and align with the Mission's Statement of Faith.

YOUR TOP RESPONSIBILITIES

  • Conduct phone screens to determine prospective clients' eligibility for the RAP Program, gathering detailed information to support appropriate placement.
  • Complete intake appointments, including obtaining required documentation, releases, and consent forms while ensuring accurate and timely data entry into the Mission's database.
  • Schedule case management appointments and other follow-up services to support a smooth transition into care.
  • Conduct property searches and urine analyses in accordance with program policies and procedures.
  • Connect clients with appropriate community resources and coordinate external referrals when program eligibility requirements are not met or different levels of care are needed.
  • De-escalate crisis situations and communicate effectively with clients, families, and team members to promote positive outcomes.
  • Build collaborative relationships with community providers and internal teams to ensure continuity of care and service delivery.
  • Demonstrate understanding and comply with all relevant legal, regulatory, and compliance requirements, company policies and procedures, and the Code of Ethical Conduct.
  • Follow the Mission’s Statement of Faith and lifestyle and adhere to the Mission’s employee policy manual and guidelines set forth by the CEO.
  • Lead or participate in devotions, prayers, or other organizational activities that promote a credible Christian witness and lifestyle.
  • Perform other duties within scope as assigned.

Requirements

WHAT YOU SHOULD HAVE
  • High school diploma or GED required. Bachelor's degree from an accredited university or college in social work, sociology, psychology, counseling, or a related social service field preferred.
  • Minimum of one (1) year of experience in social services, intake, behavioral health, customer service, or a related field required.
  • Previous experience in intake, scheduling, and coordination of care is strongly preferred.
  • Certification in Peer Support is highly desired.
  • Proficient in Microsoft Office software.
  • Program database experience preferred.
