The Pennant Group, Inc. is hiring an Intake Coordinator to manage the patient intake process, communicate with patients and families, and coordinate with home health and hospice referrals in Cortez and Southern Colorado.

You will oversee insurance verification and authorization, maintain start-of-care packets, and ensure compliant admission of referrals while building positive relationships with referral sources and healthcare providers.

Qualifications

  • The Intake Coordinator must have healthcare experience, preferably in referrals/intake in a home health or hospice environment.
  • A nursing background is preferred but not required.
  • Demonstrates good communication, negotiation, and public relations skills.

Responsibilities

  • Oversees daily patient referral and intake operations and implements intake policies.
  • Ensures compliance with state, federal, and Joint Commission requirements.
  • Monitors and processes incoming faxes and physician orders.
  • Coordinates start of care packets and input into the HCHB system.
  • Verifies and scans start of care packets with required documents for compliance.
  • Maintains relationships with referral sources and coordinates care with multiple providers.
  • Oversees insurance verification, authorization, and follow up to maximize third‑party reimbursement.
  • Collects referral statistics and supports continuous quality improvement.

Job description

The Intake Coordinator is responsible for managing all aspects of the patient intake process including communicating directly with patients and families, data entry, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, facilitating a daily intake stand up meeting, and managing the insurance verification and authorization processes.

II. DUTIES & RESPONSIBILITIES
  • 1. Ensures all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
  • 2. Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
  • 3. Monitor and process all incoming faxes
  • 4. Monitor and track all home health and hospice physician orders.
  • 5. Track and process face to face packets
  • 6. Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
  • 7. Creates and maintains and keep up to date start of care packets for RN’s and Therapists
  • 8. Reviews, organize SOC documents, verify signature and input into HCHB system
  • 9. Verify and scan start of care packets with all required documents necessary for compliance
  • 10. Ensures input of new referrals as directed
  • 11. Establishes and maintains positive working relationships with current and potential referral sources.
  • 12. Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
  • 13. Ensures maximum third-party reimbursement through direct oversight of insurance verification, authorization processes, and follow up.
  • 14. Gathers, collates, and reports referral statistics including key customer referral trends.
  • 15. Maintains comprehensive working knowledge of Hospice and Home Health contractual relationships and ensures that patients are admitted according to contract provisions.
  • 16. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by PASCO/SW Healthcare.
  • 17. Attends and Participates in staff, education, and agency committee meetings as appropriate
  • 18. Assists in all therapy and skilled nursing referrals and coordination
  • 19. Participates actively in the Quality Improvement plan established by the agency
  • 20. Process and ensure delivery of requested medical records.
  • 21. Assist and process daily, all clinical workflow flow functions.
  • 22. Performs other duties as assigned.

Starting Salary $19.00 - $ 25.00
