Job summary

PASCO/SW Home Health & Hospice in Colorado is seeking a Licensed Practical Nurse (LPN) to provide hands-on patient care under RN supervision, ensuring comfort, safety, and positive outcomes in home settings.

The role requires CO LPN license, CPR certification, and a preference for 3 years in home health or med-surg. A reliable vehicle and drive-time flexibility are expected.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN/LVN license in Colorado.
• 3 years nursing experience preferred (Home Health or Med-Surg a plus).
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver compassionate, direct patient care in accordance with state regulations.
• Implement care plans developed by the RN and document progress accurately.
• Monitor patient conditions and communicate observations to the RN and physician.
• Collaborate with the care team to ensure seamless, high-quality service.
