Candidate Experience site is seeking a Supervisor I - Intake and Assessment in Reading, PA. This role supervises staff, manages workload, and maintains relationships with service providers to ensure timely assessments and referrals.

The position requires knowledge of aging services, strong leadership, and adherence to confidentiality and regulatory requirements. Flexibility for hybrid work and occasional travel is expected.

Qualifications

• Bachelor's degree in Social Work or related field with 12 college hours in social sciences AND 1 year social work experience.
• Or 2 years of case work including assessment experience.
• Equivalents accepted; 1 year AAA experience may substitute for 1 year assessment.
• Certification by PA as a certified assessor within 90 days of employment.
• Valid state driver's license.

Responsibilities

• Supervise Intake and Assessment Unit staff.
• Develop knowledge of aging services in Berks County and Commonwealth.
• Oversee intake calls, referrals, level of care assessments, and reports.
• Direct scheduling and processing of referrals and assessments.
• Track and report assessments for completeness and compliance.
• Lead performance evaluations and staff training needs.
• Maintain HIPAA confidentiality and ethics compliance.
• Provide back-up support to other units and supervisors.
• Participate in outreach and training with partner agencies.
