UCI Health in Orange County seeks a clinical nurse to collaborate with interdisciplinary teams, delivering comprehensive care to assigned patients. You will assess needs, plan care, intervene, and advocate for patients across the care continuum, including discharge planning and referrals.

You will mentor students and newer nurses, functioning as a Patient Care Coordinator as needed, while maintaining high-quality patient outcomes in a dynamic environment.

Qualifications

• Minimum of 2 years telemetry experience.
• Minimum of 2 years inpatient adult acute care nursing.
• All external candidates must have a BSN.
• Ability to work variable shifts including evenings, nights, weekends and holidays.
• Ability to read, write and communicate effectively in English.
• Ability to adapt in a fast-paced, evolving environment.

Responsabilités

• Collaborate with interdisciplinary teams to develop and implement care plans for a specific patient group.
• Assess, plan, intervene and advocate for patients throughout the continuum of care and discharge planning.
• Evaluate patient outcomes and adjust plans; refer as needed.
• Serve as a role model and mentor to students and newer staff; function as a PCC when requested.
