Job summary

UCI Health in Orange, CA is seeking a Clinical Nurse to join our team as a Patient Care Coordinator. You will collaborate with interdisciplinary teams to develop and implement a plan of care for a specific patient group, assess needs, and advocate for patients throughout the continuum of care.

The role requires a BSN, at least 2 years of acute care nursing with telemetry experience, flexibility for variable shifts, and strong English communication.

Qualifications

• Minimum of 2 years acute care nursing experience, with at least 1 year of telemetry experience or completion of UC Irvine Medical Center Clinical Nurse I SSDU Program.
• Bachelor of Science in Nursing required for external candidates.
• Ability to work variable shifts including evenings, nights, weekends, and holidays.
• Ability to read, write and communicate effectively in English.
• Ability to maintain flexibility and work well in a fast paced, constantly changing environment.

Responsibilities

• Collaborate with interdisciplinary teams to develop and implement a plan of care for a specific group of assigned patients.
• Perform professional nursing duties to assess, plan, intervene and advocate for the patient across the continuum of care.
• Evaluate patient outcomes and readiness for discharge; revise plan or make referrals as necessary.
• Act as a role model and mentor to students and nurses new to the unit; function as a PCC when requested by supervisor.
