Capital Health is seeking a Speech-Language Pathologist to assess and treat patients across hospital, outpatient, and home-based settings. You will plan and deliver therapy, document outcomes, and coordinate care with physicians and the care team.

Licensed SLP in NJ, with 1 year of relevant experience preferred, and ongoing continuing education are expected. Telecommuting eligible, with a strong focus on patient-centered care and multidisciplinary collaboration.

Qualifications

• Graduation from an accredited speech-language pathology program or equivalent approved by the NJ State Board.
• Licensed as a speech-language pathologist by the New Jersey Division of Consumer Affairs, Audiology and Speech-Language Pathology Advisory Committee.

Responsabilités

• Assess and treat patients in hospital, outpatient clinic, and home settings.
• Develop measurable treatment plans and implement therapy.
• Coordinate with physicians, nurses, case managers and other team members.
• Document evaluation, care, attendance and discharge accurately and timely.
• Participate in professional development, PI activities and community presentations.
